• PHYSICAL FORM

  An ACA Physical Form is required for all Student Athletes participating in sports before they may attend practice or games (3rd through 12th Grades).

• PHYSICIAN'S STATEMENT

  Current Physician's Statement is required on or before the first day of school for all students enrolled at Little Sprouts through 2nd Grade at ACA.
